01. Form to be filled online only.
02. Date of birth of the Child must be between **01.01.2023 and 31.12.2023 (both days inclusive). Date of Birth should be strictly followed and the Birth Certificate from the Govt. / Gram Panchayat should be produced while submitting the form.
03. Parents are requested to take print out of duly filled online application form and submit to the office at the given date and time along with the following documents. (Submission date and time will appear on the Application Form in PDF format. Handwritten Application will not be accepted)
04. While submission please attach:
i) One attested copy of Birth Certificate of the child. [Original to be brought for verification]
ii) One attested copy of Address Proof
iii) Recent Photograph of the Child to be affixed on the application form.
05. The child must accompany at least one Parent for verification and submission of the documents on the given date.
06. Cost of Application form Rs.200/- to be paid at the time of form submission.
07. Time schedule should be strictly followed.
